Traction? Pumps? PRP? Tadalafil? Which Actually Works, and Which Is a Waste of Money?

Together with my medical writing colleagues, we spent 8 weeks reviewing the evidence-based medical literature across more than 300 papers and 12 research dimensions, to give you a no-nonsense, data-only answer.

Let's start with the conclusion —

✅ The only method that truly "lengthens" is traction (PTT). The RestoreX device from Mayo Clinic conducted a randomized controlled trial of 110 people: 30–90 minutes of daily traction for 3 months yielded an average gain of 1.5 cm, and up to 2.3 cm at 6 months, with a 95% response rate and zero serious side effects. This is currently the only method backed by high-quality RCT evidence.

❌ Vacuum pumps (Bathmate-type) cannot enlarge a normal penis. The only RCT specifically measuring enlargement (37 participants, 6 months of pumping) found no statistically significant difference. The conclusion was blunt: "vacuum therapy is not an effective method of penile lengthening." Its real value is preventing shrinkage after prostate surgery — not growth for healthy individuals.

❌ PRP (P-Shot) has zero enlargement RCTs. Over 680 clinics worldwide promote it, often charging thousands of dollars per injection, claiming it can "add 1–2 inches." But as of 2025, no randomized controlled trial has used penile size change as an endpoint. The SMSNA medical society's position: limited to clinical trials only, not recommended for routine use.

❌ Tadalafil is for "preventing shrinkage," not "enlargement." A daily 5 mg dose does reduce post-surgical shortening by about 4 mm, but this is loss-prevention, not a gain. The effect disappears once the medication is stopped.

On combination therapy (traction + pump + PRP + tadalafil):

The P-Long study (using all four together) reported a +2.04 cm result at 6 months — looks promising, right? But here's the problem: that result falls entirely within the range of traction alone (1.5–2.3 cm). In other words, you might spend thousands on PRP, a pump, and medication, only to end up with the same outcome as someone who simply bought a $500 traction device.

It should be noted that the current research has significant limitations — for example, small numbers of participants, inconsistent devices used, and so on. It must also be acknowledged that research on enlargement worldwide remains scarce. In my personal experience, combining a pump with traction and tadalafil offers meaningful benefit, even though the summarized studies do not reflect this. So medicine and clinical trials still have many limitations; please view the findings with appropriate caution.
